Referral Support

New integrated healthy lifestyles service in Gloucestershire from January 2017

The council has commissioned ICE Creates to deliver the new service which will provide stop smoking, weight management, alcohol reduction and physical activity support services (previously provided separately) through this one integrated service.

As from 1st January 2017 all referrals to the healthy lifestyles service should be made through the new single point of access, which is: tel: 0800 122 3788 or email: info@hlsglos.org

Note also that the weight management referral pathway will remain the same until 31st March 2017 (referral into Slimming World). Specialist stop smoking services do not continue but Pharmacy Stop Smoking services do continue – we will update this page with recording information if there is a change.

The council will notify any subsequent changes well ahead of them coming into effect.

Village and Community Agents

 As HLPs, you have been tasked by the Health & Wellbeing Board to make contact with your Village/ Community Agents to explain the concept of HLP programme and how you can support their client group.  You can use the HLP leaflet to support this discussion, and include the services you provide from your pharmacy.

 What are Village/ Community Agents?

This scheme has been developed to help older people in rural Gloucestershire and older members of the BME communities across the county to feel more secure, more cared for, and have a better quality of life.

 It is designed to give older (50yr+) people easy access to a wide range of information that will enable them to make informed decisions about their own well-being and help them remain in their own homes for as long as possible – supported by an enabling community.

Agents are trusted members of their communities, people who can be approached for advice and support.  Through training and access to appropriate information resources, the Agents provide both a short and a longer term service. This is an opportunity for pharmacy to support with medicines management and self-care advice.

Health Trainers

The Community Health Trainer Referral Form allows you to signpost customers to Health Trainers, who have the skills to support local residents to make healthier lifestyle choices, like losing some weight, getting more active or eating healthily. The service is free and confidential.

Let’s Talk

Let’s Talk is a free local service from the NHS that helps people overcome the common mental health difficulties. We aim to help the customer to understand themselves better and make changes to the way they think and behave so they can feel better and ‘more like themselves again’. Services may be offered by referral through the family surgery or self-referral through a free phone telephone number or course booking website.

A trained therapist is at the end of a phone and will listen to the customer’s needs, then support and guide them through the Let’s Talk service. We can offer help through:

  •  just one or two telephone calls
  • self help reading through our Books on Prescription service at local libraries
  • a wide range of 5 week courses covering low mood, IBS, OCD, anxiety, stress etc plus an introductory emotional wellbeing course
  • an individual therapy or programme

Whatever the journey with us, a member of our team will stay in touch with the customer and guide them through what can be a difficult time in their life.

Fire safety elearning course

This course will highlight how the local fire and rescue services (FRSs) work in conjunction with primary care to reduce the risk of injury and death amongst the patient population, particularly vulnerable groups. It will help you identify people who are more vulnerable to the risks of fire in the home, and provides information on how GPs can refer vulnerable people to the FRS for a home fire safety visit.

Multi-Agency Hoarding Guidance
This
document sets out a framework for collaborative multi-agency working within Gloucestershire using a ‘person centred solution’ based model.
